Angiokeratomas are benign vascular lesions caused by dilatation of superficial dermal capillaries with secondary epidermal hyperkeratosis. Although angiokeratoma of Fordyce commonly involves the scrotum, localization to the penile meatus is extremely rare and may mimic sexually transmitted infections or malignant lesions. We report a rare case of angiokeratoma involving the penile meatus presenting as a few clustered lesions, highlighting its clinical features, diagnostic considerations, and management.
